The PCOM Act 48 Request Form is to be used by professional educators who have successfully completed continuing education credits at Philadelphia College of Osteopathic Medicine.
Act 48 requires that every five years, all certified educators earn six college credits, six continuing professional education credits, or any combination of collegiate studies, continuing professional education courses or learning experiences equivalent to 180 hours. Philadelphia College of Osteopathic Medicine has been authorized to submit to the Pennsylvania Department of Education graduate credit courses only.
REGISTRAR REQUEST FORMTo apply for Act 48 credits, please complete our online Registrar Request Form. Your Act 48 credit(s) will be submitted to the Pennsylvania Department of Education within 5-7 business days. You will receive a confirmation letter within 7-10 business days.
