State Licensure

Certifications for state licensure must complete our online Registrar Request Form and select the "Licensure" form request.

Procedure for Licensing

  • Submit the form from the state you are requesting licensure.
    • Be sure that your name is noted on the form.
    • To find licensure forms, go to the website of the state board where you are requesting licensure and search for the board's “verification of medical education form."
  • If a transcript is required to be submitted with the form, please also complete our online Registrar Request Form and select "Transcripts."
  • Attach any instructions that the state provides.
  • Email your documents to licensure@pcom.edu.
    • Be sure to include your phone number in the email.
