PCOM requests that you complete this form to provide the information needed to prepare for graduation.

Instructions for completion:
Due to the fact that this form must be completed all at one time, we have listed some information that you will be required to have in order to complete the form.
Required Data are the fields that are visible on the form. The required data includes:
- Your PCOM email address - mandatory
- Degree Program at Philadelphia College of Osteopathic Medicine
- Name as you want it on the diploma
- Hometown - where you would like to be identified as from ** If you are not from the United States, you can enter your City and Country in the City field on the form **
- Your address - where you are currently living
- For cap and gown order: Height, Weight and approximate suit or dress size
- Prior degree information is required for the commencement booklet, and therefore you need to provide (you can list one undergraduate and up to two graduate institutions from which you received a degree)
- Name of Institution
- Year degree granted
- Degree (ex: BA, BS, MS, other)
- Degree major
