PCOM requests that you complete this form to provide the information needed to prepare for graduation.

DO Graduates
NOTE: After the match, you will be sent an e-mail from your SGA representative asking you to provide information as to where you matched and your specialization. This information is needed to be completed immediately upon receipt so that the Commencement booklet can be prepared.

Instructions for completion: All graduates
Due to the fact that this form must be completed all at one time, we have listed some information that you will be required to have in order to complete the form.
The required data includes:
- Your PCOM email address - mandatory
- Name as you want it on the diploma
- Name as you want it in the program for DO graduates only
- Name as you want it announced for DO graduates only
Home town - where you would like to be identified as from
** If you are not from the United States, you can enter your City and Country in the City field on the form **
Your address - where you are currently living
For cap and gown order: Height, Weight and approximate suit or dress size
Prior degree information is required for the commencement booklet, and therefore you need to provide:
(You can list one undergraduate and up to two graduate institutions from which you received a degree)
Name of the institution
- Year degree granted
- Degree (ex: BA, BS, MS, other)
- Degree major
Publicity information
An opportunity to provide your information for a press release will be available at a later time.
