PCOM - Change Address or Name

Address Change

Currently enrolled students must update their address (mailing and billing) on-line at Nucleus Web Services.

Inactive students without an active Nucleus Web Services account must complete a

Change of Address Form.

Alumni wishing to update their address should contact the Alumni Relations and Development Office.

Name Change

All students reporting a name change must complete a Change of Name Form. It is important that changes in name are reported in a timely manner.

Alumni wishing to update their name should contact the Alumni Relations and Development Office.

Obtain a Change of Address or a Change of Name Form through one of the three options:

Option 2: Pick-Up at the Office of the Registrar.

Option 3: Call or E-mail the Office of the Registrar and request the form be mailed or faxed to you.
